HOW BILLING WORKS
$60/month per athlete. Payment is due on the 1st of each month. We will reach out with a reminder a few days before each billing date.
First month free if joining after the 5th. If you sign up after the 5th of the month your first full $60 payment begins on the 1st of the following month.
Month-to-month — no contracts. Cancel anytime with at least one full month advance notice before your next billing date so we can open the spot for another athlete.
No refunds once a monthly payment has been processed, regardless of attendance.
Multiple payment options available including cash, Apple Pay, and more. We will follow up after registration with payment details and a secure payment link.
LIABILITY WAIVER & RELEASE
Assumption of Risk: I understand that participation in athletic training activities involves inherent risks, including but not limited to muscle strains, sprains, and other physical injuries. I voluntarily accept these risks on behalf of my child.
Release of Liability: I hereby release Legendary Fitness, OneEighty Athletics, their coaches, staff, and facility owners from any and all claims, demands, and causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained by my child while participating in this program or while on the premises of 1121 E Poplar St, Fort Gibson, OK.
Medical Authorization: In the event of an emergency, I authorize Legendary Fitness staff to seek appropriate medical attention for my child. I agree to be responsible for all medical costs incurred.
Code of Conduct: I agree that my child will conduct themselves respectfully toward coaches, staff, and fellow athletes. Legendary Fitness reserves the right to dismiss any participant whose behavior is disruptive or unsafe, with no refund of fees paid.
Health Disclosure: I confirm that my child is in adequate physical health to participate in this program and I have disclosed any relevant medical conditions or physical limitations in the notes field below.
This waiver is binding upon myself, my child, heirs, assigns, and legal representatives.
PAYMENT ACKNOWLEDGMENT
• Monthly fee is $60 per athlete, due on the 1st of each month
• If joining after the 5th of the month, first payment begins on the 1st of the following month
• This is a month-to-month program with no long-term contract
• Cancellation requires at least one full month advance notice before the next billing date
• No refunds will be issued once payment has been processed, regardless of attendance or circumstances
• Failure to cancel in advance will result in the final month being charged in full
